Student Health Records

For the health and safety of our campus community, the Student Health Center must have an up-to-date health history for each student that includes information such as food/drug allergies, medical conditions, medications, etc.  All new and transfer students are required to submit the mandatory health forms prior to move-in or the start of classes.

Deadlines for Records

Fall Deadline - Aug. 1

Spring Deadline - Jan. 1 

Failure to submit a completed Student Medical Report Form AND a completed Student Immunization Record may result in the student's inability to register for classes. To request a waiver of the immunization requirement, students must complete the Immunization Exemption Form below.

Submit all completed forms to the Student Health Center in person, by mail, by email, or online via PointSync.

Requesting Records

Student health records are kept for 7 years from the date they become inactive.  Inactive records result from graduation or withdrawal.

For any record requests, please download the Record Release Form.  The form must be emailed in PDF format to studenthealth@pointpark.edu along with your full name, date of birth, and year of enrollment.  Please allow 5 business days for your records to be located in storage.  You will receive and email back to make arrangements for the delivery of your records.
